NEW STUDENT ENROLLMENT PROCESS
If your student attends another building in the Oxford School District, you do not need to complete the following process. Please contact your current principal or counselor to begin an in-district transfer.

Health Appraisal Form
DK & Kindergarten: Physician to perform a vision and hearing screening along with the health appraisal/physical and provide a signature; (hearing screening can be obtained by local health department if your doctor does not provide this service)
Dental screenings will also be required as of 1/1/2024.
Parents are to complete the top portion of page 1 (PERSONAL and SECTION 1 - HEALTH HISTORY) then provide a signature and date.

Grades 1 - 12: Parents are to complete the top portion (PERSONAL and SECTION 1 - HEALTH HISTORY) then sign and date; no doctor's appointment is necessary.

Original Birth Certificate

Parent/guardian driver's license showing the correct address (matches proofs of residency). The ID cannot be expired.
Two Proofs of Residency
ALL STUDENTS
Residency documentation must be dated within 30 days of enrollment and you must provide the following:

If you own your home (#1 and #2 Required):

Current Mortgage statement OR current property tax statement (Summer/Winter) with your first & last name, address and current date.

A current bill (utility, wireless, insurance bill or bank statement) with your first & last name, address, and current date.

If you rent (#1 and #2 Required):

Current lease (all pages) with your first & last name and address, landlords name, phone number and term date of the lease, signatures of Lessor and Lessee, dated (cannot be expired).

A current bill (utility, wireless, insurance bill or bank statement) with your first & last name, address, and current date.
If you are living with a resident (#1 - #3 Required):

Current mortgage statement or property tax statement OR lease of the homeowner (all pages) with their first & last name, address, and current date.

A current bill (utility, wireless, insurance bill or bank statement) with your first & last name, address, and current date.

A signed and notarized affidavit by you (the parent) and the homeowner. Oxford Schools will supply this document and can notarize at no charge.
Immunization Record or Waiver
ALL STUDENTS
Either a complete and up-to-date immunization record from doctor or health department, or a waiver from a county health department is accepted.

Immunization disclaimer: If your child’s school/state-required immunization record is in an “incomplete” status on November 1st and February 1st, they will be excluded from attending physical locations until the immunization record is moved to a “complete” or “provisional” status, or waivered. Your child’s immunization record may be complete at the time of enrollment, however, it is a living record that must be updated. Please seek guidance from your physician if you have further questions.
