*Either a complete and up-to-date immunization record from doctor or health department, or a waiver from a county health department is accepted. If providing a waiver, it must be made out to OVA and dated within the school year the student will be taking classes with us.
Immunization disclaimer: If your child’s school/state-required immunization record is in an “incomplete” status on November 1st and February 1st, they will be excluded from attending physical locations until the immunization record is moved to a “complete” or “provisional” status, or waivered. Your child’s immunization record may be complete at the time of enrollment, however, it is a living record that must be updated. Please seek guidance from your physician if you have further questions.
*We understand that due to COVID-19, many health-related offices are closed or allowing limited access. We highly recommend attempting to schedule your appointment(s) now, for a later date, at either your Doctor's office (Immunizations) and/or local health department (Waivers) in order to obtain the required documentation for enrollment.
